LEVEL 2 EDUCATION ONLINE INTAKE: 

If you have not started and/or not completed Level 2 Alcohol Education classes yet, you will start HERE with Education. ​
​(Do not register for Level 2 Therapy at this time.)
THREE EASY STEPS to complete the online intake.
This ONLINE INTAKE will take approximately 45 minutes to complete.
Please plan accordingly in order to complete in it's entirety.

LEVEL 2 ALCOHOL EDUCATION ​REGISTRATION
You will complete 4 documents: Registration, Telehealth Consent, Treatment Contract and Assessment.
MAKE PAYMENT
You will select 1 of 3 options for payment.

​OPTION 1. Intake ($75 + $30 for first class) = $105 OR
OPTION 2. Intake + Affidavit of Enrollment ($75 + $60 for first two classes) = $155 OR
OPTION 3. Pay in full for discount of 10% ($75 + $360-10%) = $399
Classes are $30/ class or $27/ class with discount. Affidavits of Enrollment ($20) FREE when PAID in FULL.

​*ZOOM Log in instructions for groups are sent the evening before class.
Unless PAID IN FULL, "pay per class" invoices will also go out the night before class. Pay online.
